A leading medical center in State College, PA is seeking a dedicated Clinical Pharmacist to join their team on a direct hire basis. This hybrid role blends clinical expertise with staff pharmacist responsibilities to provide comprehensive patient care and support hospital operations.
In this position, you will spend approximately 60% of your time focusing on clinical work with an emphasis on Population Health and Ambulatory Care. You will manage medication regimens for patients, particularly those with chronic conditions such as diabetes, collaborating closely with healthcare teams to optimize patient outcomes. The remaining 40% of your shift will involve traditional staff pharmacist duties, balancing the needs of a smaller hospital setting. There is strong potential for this role to evolve into a full-time clinical position as the clinical program expands.
Responsibilities
- Conduct clinical evaluations and manage medication therapy with a focus on target disease states.
- Collaborate with multidisciplinary teams to improve population health outcomes.
- Provide staffing support including dispensing and medication safety oversight.
- Utilize Meditech for patient and pharmacy information management.
- Support the growth and development of clinical services within the institution.
Qualifications And Desired Experience
- Active Pennsylvania pharmacist license.
- Experience in ambulatory care or inpatient pharmacy settings preferred.
- Completed pharmacy residency is highly desirable.
- Passion for clinical practice and patient-centered care.
- Ability to work variable day and afternoon shifts, primarily 8-hour day shifts.

This opportunity offers a competitive salary range of $106,000 to $165,000 per year, with a midpoint around $135,000.
